Can your doctor become my primary care physician?
Yes. We hope you will value the ease and convenience of our onsite medical services and allow us to be your primary care provider.
Do I need to switch to your doctor in order to have access to the clinic?
No. You maykeep all your doctors and specialists and still use the clinic for acute care needs.
What are the benefits of using the onsite clinic as my PCP?
Along with the convenience of having providers onsite to handle your annual physicals, vaccinations, medications, and other health care needs, we work closely with all areas of the community to ensure there is a continuity of care for our patients. Whether you need rehab, a specialist referral, or are transferring to assisted living, our providers can help. There is no need to switch providers when transitioning to Assisted Living as our doctors and nurse practitioners are credentialed to continue your care.
What do I need to bring with me to my first appointment?
You will need to present your insurance card(s), ID, a complete medication list, and a form of payment for any co-pays or co-insurance amounts that may be due at the time of your visit.
Will x-rays and other diagnostic services be available onsite?
Yes. In most cases, tests can be done the same day.
What is the process for medication refills?
We ask that you give us 48-72 hours notice for pharmacy medication refills requests. For mail order prescriptions, we ask that you allow us up to 2 weeks to set up delivery. Please be sure to provide updated information regarding your prescription coverage(s) and preferred pharmacies.
Will the doctor visit me in the hospital?
Our clinicians will coordinate transfer of care to the hospital and will be prepared to see you when you return home.
What do I do if I need a doctor on the weekends or after hours?
Like any doctor’s office, we have set business hours. Please use your best judgment if immediate medical attention is required by calling 911 or utilizing your emergency device. For messages left regarding appointments and non-urgent care matters, our staff will return your call the next business day. If it is not an emergency, please call our office and leave a message. Our Director of Patient Care will receive messages and calls after hours and on the weekends.
How do I know if you accept my insurance?
We accept Medicare, Medicare Supplement Plans and major commercial plans, such as, BCBS, UnitedHealthcare, Aetna, AARP, UMR, Tricare, Humana, and Cigna. For more information about insurance, please visit our clinic and we can assist you.
Is there a cost to use your clinic?
There are no membership fees to use our services. We will file with your insurance company. However, you will be responsible for normal co-pays and co-insurance amounts.
What forms of payment do you accept?
All major credit cards such as Visa, MasterCard, and American Express, along with debit cards and cash or check.
Do you have cash pay rates?
Yes. For those patients who do not have insurance or would like to pay out-of-pocket, we have cash pay rates available.
