AveoMedical is a new patient-centered care platform for senior living. We deliver mobilized primary care services that are preventive and technology enabled through telehealth and an advanced patient portal to bring a higher level of service and communication to patients, providers, caregivers, and families. Our providers have a better work life balance and can actually build lasting relationships with their patients for healthier lives. We strongly believe that the US healthcare system is broken and we need to start by changing the way it is delivered, from a volume-based system to a value-based system. Primary Care physician
North DFW - DALLAS / PLANO

Requirements:
Internal Medicine, Family Practice, or PM&R Board Certification
Experience managing geriatric patients in an ambulatory/clinic setting
Ability to travel to Senior Living Facilities in the North DFW area (Dallas / Plano)
Ability to supervise mid-level providers
Part time - direct patient care, 3-5 days per month (estimated slow grow over time)
Comfortable using Electronic Health Records (experience with Athena is a plus)
Experience working in Senior Living Facilities is a plus

RESPONSIBILITIES:
Direct patient care 3-5 days per week at our Senior Living Community clinics (Presbyterian Village North, Legacy Midtown Park, Legacy Willow Bend) with an emphasis on preventive care.
Supervising Mid-level Providers and Patient Care Coordinators/Medical Assistants.
Light Medical Directorship Responsibilities, including periodic meetings with administration, team building exercises, and quarterly health education presentations to our patient populations.
Oversee quality & growth of Chronic Care Management program, as well as other clinically integrated services.
Coordinate care plans & sign-off charts in a timely manner. Manage professional relationships, gain rapport, & lead care teams to execute care plans.
Maintain availability throughout the week to field calls, emails, & texts from the Patient Care Coordination team to quarterback care plan execution & or respond to more urgent patient needs.
Technologically savvy to use EHR, care coordination, telemedicine, & patient portal technologies to take action on insights through data, video, & audio to ensure patients have adequate & timely access to care.

Responsibilities:
Take comprehensive histories and provide physical examinations and other health assessment and screening activities.
Diagnose, treat, and manage patients with acute and chronic illnesses and disease.
Order, perform, supervise, and interpret laboratory and imaging studies.
Prescribe medications, durable medical equipment, and advanced therapeutic interventions.

Patient Care Coordinator (Clinic Lead)

Geriatric/Adult Care Coordination, Case Management, & Medical Office backgrounds are preferred; Strong in Back Office clinic functions;
Time requirements:
30 hours per week; Opportunity to ramp to 40 hours as position matures.
Monday - Friday 9a-3p.
Responsibilities:
Lead the clinic to facilitate high-level triage and coordinate patient, resident, and community care requests.
Serve patients by greeting and assisting them with utmost customer service; triage phone calls, schedule appointments, coordinate provider schedules, verify benefit eligibility, maintain records and patient accounts.
Proficient with back office functions is key; Take vitals, phlebotomy, wound care, light suturing, IVs, patient triage, and adequately maintain patient check ins and outs during provider clinic days.
Orient and educate patients and their families by meeting with them, explaining the role of the patient care coordinator, initiating the care plan, and providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements.
Obtain medical records when requested by a provider.
Develop interdisciplinary care plan and other case management tools by participating in meetings, coordinate information and care requirements with other care providers, resolve issues that could affect smooth care progression, foster peer support, and provide education to others regarding the case management process.
Monitor delivery of care by completing patient rounds, document care, identify progress toward desired care outcomes, intervene to overcome deviations in the expected plan of care, and review the care plan with patients in conjunction with the direct care providers.
Interact with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults; report personnel and performance issues to the unit manager, maintain ongoing communication with utilization review staff regarding variances from the care plan or transfer/discharge plan.
Evaluate outcomes of care with the interdisciplinary team by measuring intervention effectiveness; implement team recommendations.
Respect patients by recognizing their rights; maintain confidentiality.
Maintain quality service by establishing and enforcing AveoMedical’s organization standards.
Maintain patient care database by entering new information as it becomes available; verify findings and reports, and back up data.
Review and analyze patient data reports (e.g., discharges, hospital admissions, read missions, skilled nursing facilities). Follow up as necessary.
Revenue cycle management, coding, and proficient with primary care EHR administrative, authorization, and charting functions.

TIME REQUIREMENTS:
Part-time position with flexible working hours. Hours are from 9AM to 2PM, three to five days per week, Monday to Friday.
During COVID-19 pandemic, employment must be exclusive with Aveo Medical - the facilities in which we operate do not allow for moonlighting during COVID-19.
RESPONSIBILITIES:
Primary duties involve administering daily clinic activities and supporting the clinical team under supervision of Patient Care Manager.
Cheerfully meet and greet patients, serving as the first point of contact.
Verbal communications delivered with high EQ in a concise and confident manner.
Professionally answer phone calls in a timely manner.
Schedule (New, Existing, Follow-Ups, Special Examinations, Walk-ins) patient appointments.
Cross trained with back office functions, such as CLIA-waived testing.
Proficiently build and analyze clinical reports from EHR for patient outreach to schedule follow ups for preventive services like Annual Wellness Visits, Chronic Care Management, Care Plan Oversight, Diabetes management, etc.
Work with patients’ insurance companies to ensure coverage & facilitate referrals via authorizations. Collect co-payments or deductibles, demographic information, signed consent forms, and insurance information.
Scan, file, and organize patient documents in EHR with attention to detail to ensure complete and accurate information.
Experience with medical billing and coding.
Exhibits high character and strong work ethic and works well within a team environment.
Proficient in Google Suite, i.e. Docs & Sheets, as well as MS Office.
